Utah canyon BASE jump kills 2, including extreme athlete who performed with Madonna

Two people, including extreme athlete Andy Lewis, were killed in a BASE-jumping accident at Mineral Bottom in a Utah canyon.
Lewis gained widespread attention after performing slackline tricks onstage with Madonna during the 2012 Super Bowl halftime show.
He ran BASE Jump Moab and was a celebrated slackliner — a four-time world champion and Guinness record holder known for daring stunts like walking a line between hot-air balloons.
