Man mistakenly prescribed morphine died two days later from overdose

A man was mistakenly sent home with morphine and died two days later; the Public Services Ombudsman called it an "extremely serious injustice."
The ombudsman's report blamed a series of medical and pharmacy checks that failed and noted the patient wasn’t given proper advice about opioid risks.
